New Student Information

If you are new to Davis School District or your child will be a new student attending Taylor Elementary School in the Fall of 2020, please fill out the following information:

In order to register your child, please bring the following:

1. Student Information Form (This is a fillable form, which means you can type in your information and then print it out and bring with you)

2. Birth Certificate- Present school with original birth certificate a copy will be made for our records.

3. Immunizations- Provide school with student immunization record.

4. Proof of Residency- Provide two proof of residency (must be dated within the last 60 days). 

5. Guardianship Status Form- Will be asked to show valid I.D.

6. Housing Information/ McKinney-Vento

7. Request for Student Record Form
